RACE TO THE PYRAMIDS


Over at Church Street Medical Centre we have extremely big hearted GPs, though a little wacky!!!  Three of them have very kindly decided to raise money for Eastwood Memory Cafe with a totally amazing challenge.  I would be really grateful if you could share this with anyone and everyone you know, so we can hopefully reach the target set.

In order to support the amazing work of Eastwood Memory Cafe; we have set ourselves the challenge of collectively running the equivalent distance of Eastwood to The Pyramids of Giza, 2200 miles, by the end of 2018 and would love to raise the same number in £s - £2200!!!

Please support this fabulous charity which makes such a difference to those affected by dementia in our local area.

Feel free to keep nagging us to get those running miles in. We will give monthly updates with progress toward our target!

Eastwood Primary Care Centre

Church Street Medical Centre and Church Walk Surgery merged to form Eastwood Primary Care Centre on 31st May 2018. We are both long established GP Practices and have worked collaboratively for many years now.

Church Street site is the Urgent Care Centre for issues that require attention on the day - when calling please Dial 1. Church Walk site is the Planned Care Centre for pre-bookable routine appointments - when calling please Dial 2.

Staff from both practices will be working across both sites.

We run many clinics on site for  patients with chronic illnesses such as Diabetes, Asthma and COPD. In the community we are supported by Community Heart Failure Nurses, COPD Nurses, Community Matrons and End of Life nurses.

We are fortunate to have an Ultrasound scan service, a Specialist Pain clinic, a Gynaecology Clinic, a Counselling service and also a Memory and Dementia clinic operating from the premises.

We also offer a wide variety of other medical services such as antenatal and postnatal care, minor surgery, childhood and travel vaccinations, contraceptive implants and coils, joint injections and well-person check-ups.

The practice is a dispensing practice, a service which we can offer to our patients in certain areas of Underwood. Our dispensary team can organise dosette medication and delivery for patients who require this service.

We want to get better at communicating with our patients.

 

We want to make sure you can read and understand the information we send you. If you find it hard to read our letters or if you need someone to support you at appointments, please let us know.

 

We want to know if you need information in braille, large print or easy read.

 

We want to know if you need a British Sign Language interpreter or advocate.

 

We want to know if we can support you to lipread or use a hearing aid or communication tool.

disabled access Wheelchair access is via a ramp. Patient services are all on ground floor level with no steps. A disabled patients’ WC is available and clearly marked. Corridors and doors are suitable for wheelchair access.

 Appointments

Appointment availability within the practice is being constantly monitored to try and create the right balance between appointments released on the day for acute illnesses and bookable in advance appointments for patient reviews, chronic disease management and non-urgent patient care. We now have in place an Urgent Care Centre and a Planned Care Centre.

Important InformationPlease be aware that the Make Custom Order link on your repeat ordering page is to notify us of medicines you have been unable to order. If you are ordering repeat items at the same time please send this request first - we cannot provide a confirmation of order or a reminder of next prescription service.

 

Please do not ring the practice to check we have the order (the system is linked directly to our clinical system so we have your order), this not only causes delays for patients ringing to get through regarding appointments etc, but impacts on the prescriptions teams’ ability to process the 100's of prescriptions we handle daily. For prescription queries please only call between the hours of 11am - 3pm. Thank you
